Requirements to determine E&T funding eligibility (additional documents may also be required):

  • Must apply for Higher Education and receive a referral to be eligible. Employment & Training Educational Services shall be considered Secondary to all other resources.

  • All students must complete a FAFSA application and submit it to their educational institution.

  • Enrollment Documentation Tribal ID: Certificate of Indian Blood (must be a member of a Federally Recognized Tribe)

  • Social Security Number: for each household member 

  • Written Request: Brief summary of education, graduation plan, etc. Latest earning statement, Unemployment Insurance 

  • Household Income: Latest earning statement, Unemployment Insurance Statement, TANF termination/denial, No Income verification letter

  • Verify Physical Address: WSDL, Utility bill, Rental agreement 

  • Verify Employment: Employer verifying Program unable to fund

  • Latest Transcript: Either HS, GED, or College

  • FASFA/Attachment I: Award letter. Required to determine UNMET needs

  • Degree Plan: From college counselor/advisor

  • Placement Test: From college counselor/advisor

Students must agree to

  • Acknowledge misuse of educational funds for illegal or other personal purposes not permitted; will be denied educational services for up to one year.

  • Submit a copy of their registration for class and class schedule.

  • Submit copies of grade reports within 10 calendar days of receipt, as detailed in the case plan. At a minimum, to continue services, submit grades monthly. 

  • Complete a school release of information form.

  • Submit a copy of any certification received upon completion.

  • Submit attendance sheets monthly.

  • Notify in writing of any changes to their class schedule, if they withdraw from school, or if they are put on academic probation.

  • Acknowledge students who withdraw or do not complete their course work without reasonable cause or do not maintain the required GPA will be put on sanction, and will not be eligible for Educational services or funds for a period of one quarter/semester.  Hardship cases will be considered on a case-by-case basis and remedial services may be required as a condition of continued funding.

  • The Employment & Training Services shall be considered a secondary resource to all other financial resources.  Applicants may be required to provide documentation that they have applied for and exhausted all other resources before they can utilize these services.
